Abstract

Community-based health promotion (CBHP) models offer an effective approach to cardiovascular disease prevention by addressing social determinants and supporting sustainable behavior change. CBHP relies on community participation, cultural relevance, and reducing health inequalities. The success of interventions depends on integrating behavioral change theories and involving local resources and stakeholders. Emerging technologies like mobile apps and AI enhance prevention potential, but require improved accessibility and digital literacy. Long-term impact requires strategic integration, sustainable funding, and active stakeholder engagement.
